HSP (Henoch Schonlein Purpura)

Henoch-Schonlein Purpura (HSP) is a rare, immune-mediated disease characterized by inflammation of small blood vessels, primarily affecting the skin, joints, gastrointestinal tract, and kidneys. It is the most common vasculitis in children but can occur at any age.

HSP is an IgA-mediated small vessel vasculitis. Immune complexes containing IgA deposit in small blood vessels, causing inflammation.

HSP can be triggered by:

  • Respiratory tract infections (common preceding factor).
  • Certain medications, vaccines, or allergens.
  • Autoimmune predisposition.

Symptoms

  • Palpable purpura (raised reddish-purple spots) mainly on the lower limbs and buttocks.
  • Pain and swelling, especially in knees and ankles.
  • Abdominal pain, nausea, vomiting, and blood in stool.
  • Haematuria (blood in urine) or proteinuria, indicating glomerulonephritis.

Diagnostic tests

  • Lab Tests: Elevated IgA, ESR, and CRP; urine analysis for kidney involvement.
  • Biopsy (if needed): Confirms IgA deposits in affected tissues.

Ayurveda, Henoch-Schönlein Purpura can be understood as a condition involving Raktavaha Srotas (channels carrying blood) and an imbalance in Pitta dosha (responsible for heat and inflammation) and Vata dosha (causing pain and dryness).

Ayurvedic management of HSP

Panchakarma Therapies

  • Virechana (Therapeutic Purgation): Clears excess Pitta from the system.
  • Raktamokshana (Bloodletting): Purifies the blood (can be done via leech therapy in localized cases).

Diet and lifestyle recommendations

  • Consume cooling foods: coconut water, cucumber, and ghee.
  • Avoid spicy, oily, and sour foods.
  • Practice stress-reducing activities like yoga and meditation.
  • Avoid overexposure to heat and sun.

Herbal Remedies

Manjistha (Rubia cordifolia):
  • Purifies blood and reduces inflammation.
  • Dosage: 1-2 grams of powder with warm water twice a day.
Guduchi (Tinospora cordifolia):
  • Boosts immunity and balances Pitta
  • Dosage: 2-3 grams of powder or decoction daily
Nimba (Azadirachta indica):
  • Antimicrobial and blood purifier.
  • Dosage: 1-2 capsules or neem juice daily.
Amalaki(Indian gooseberry):
  • Rejuvenates tissues and reduces Pitta.
  • Dosage: 1-2 grams of powder or fresh juice.
